Our Community Support Services team is seeking a Behavioral Health Clinician I (BHC I) This individual works in collaboration with the client in planning, implementing, and evaluating service provisions to maximize the client's independence and success in the community. BHC I completes initial assessments, prepares treatment plans, provides case management services, and documents ongoing treatment and discharge planning.

The Outpatient Mental Health Care Manager is an integral member of the interdisciplinary team involved with each individual’s care at CPC. The Outpatient Care Manager works directly with clients in clinic-based environments, community settings, and clients’ homes to coordinate services, establish goals, and support individuals in achieving wellness, recovery, and productive lives. This position involves field-based outreach to ensure access to essential services and reduce barriers to care.
The Outpatient Care Manager actively delivers case management, crisis intervention, psychoeducation, and skill-building support, directly connecting individuals to vital community resources. They work collaboratively with clients, their families, support systems, clinical teams, and community providers to achieve positive outcomes and promote long-term recovery.
As a Behavioral Health Clinician I for PACT (Program of Assertive Community Treatment), you will serve people in the community as part of a multi-disciplinary team of professionals. The PACT team works together to help adults with mental illness learn the tools to become and remain stable, healthy, and successful, living in the community. This is done through individual and group treatment modalities. The Behavioral Health Clinician provides assessments and direct services for people with serious and persistent mental illness and persons with co-occurring substance use issues enrolled in the PACT Program.
CPC's Adult Partial Care program (PC) is a community-based service that helps individuals living with serious mental illness or co-occurring mental health and substance use disorders to learn and enhance independent living skills. The Behavioral Health Clinician I provide individual, group, family, and/or substance abuse treatment for consumers who are diagnosed with co-occurring, mental health and substance abuse disorders. Works with a team to complete individual recovery planning and provides case management services. Completes Bio psychosocial Assessments and documents treatment progress in electronic clinical record.
The Mobile Crisis Sr. Program Coordinator is a Clinical Supervisor role. This opportunity is for the direct clinical supervisor of the mobile response workers and provides individual and team supervision in office and informally while out in the field. The Mobile Crisis Sr. Program Coordinator will assist staff in providing crisis intervention to clients/families and assess appropriate treatment and case management interventions and ensure that these are effectively and efficiently implemented. The Mobile Crisis Sr. Program Coordinator will monitor utilization review of IIC services to ensure that clients are receiving effective, appropriate treatment and progressing throughout treatment. The Mobile Crisis Sr. Program Coordinator will maintain relationships with community providers and maintain databases to track IIC service delivery, cost, and outcomes.
