Behavioral Health Clinician -Community Based

POSITION SUMMARY: Our Community Support Services team is seeking a Behavioral Health Clinician I (BHC.) This individual works in collaboration with the client in planning, implementing, and evaluating service provisions to maximize the client's independence and success in the community. BHC I completes initial assessments, prepares treatment plans, provides case management services, and documents ongoing treatment and discharge planning.

Location: Eatontown, NJ

MINIMAL QUALIFICATIONS:

  • Must possess terminal NJ license and Master's Degree in clinically related field such as social work or counseling.
  • Must be a LSW or LAC.
  • Must have prior experience providing treatment services to adults, children, or adolescents.
  • Experience with evidence-based practices.
  • Valid NJ Drivers License, required.

Major Responsibilities/Performance Objectives:

Some of the responsibilities include,

  1. Complete Biopsychosocial Assessment within the time frame established for the program and formulate treatment plan. Utilize DSMV to determine diagnosis and level of care. Utilize a holistic approach to complete the biopsychosocial assessment that addresses all life domains in clear and understandable language. Include client and family in the treatment planning process.
  2. Complete Risk Assessment at intake, notify supervisor if risk level changes, and monitor High Risk clients weekly.
  3. Complete Individual Recovery Plans (IRP) utilizing evidence-based treatment approaches such as Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), Stages of Change and Relapse Prevention in work with clients. Utilize a holistic approach towards developing IRPs.
  4. Provide direct services to clients and their families. Coordinate with client’s family and other agencies that provide services to client.
  5. Participate in an interdisciplinary team process to ensure the provision of services and/or linkage with requisite services to client.
  6. Record, report and monitor service delivery as per program contract requirements.
  7. Attend required meetings and training.
  8. Work cooperatively and collaboratively with other staff and supervisors.
  9. Maintain competency to provide treatment through ongoing clinical supervision and continuing education and successfully completion of goals in yearly staff evaluation.
  10. Complete clinical documentation in a timely and effective manner. Comply with regulatory, Agency and funding guidelines regarding documentation. Keep up to date on the use of Agency’s electronic health record.
  11. Follow Agency and regulatory policies regarding confidentiality and security of client information.
  12. Abide by Agency, State and Federal as well as professional licensing standards pertaining to client rights, ethical treatment, conflict of interest, HIPAA, etc.
  13. Identify High Risk clients and work with a team to provide safe and effective treatment outcomes.
