Title: Care Manager, Peer Support

The Care Manager of Peer support will provide comprehensive care management and related services for individuals enrolled in the relevant agency program. The Peer Specialist will be an integral member of an interdisciplinary clinical team that will work in collaboration with the consumer in the development and execution of a care plan that will provide a holistic approach. The Peer Specialist will work with the clinical team in assessing and identifying the needs of the consumer and providing and/or arranging for health and wellness promotion services as part of the individual recovery plan in order to maximize the consumer's successful community adaptation and quality of life.

Location: Redbank NJ

LICENSE CERTIFICATION:

Valid N.J. Driver's License.

MINIMAL QUALIFICATIONS:

  • Associates degree, knowledge of recovery-based concepts, and personal history as a consumer of mental health services.
  • Must have completed CPC approved Peer Specialist training at the time of hire or must have completed this training within one year of employment

RESPONSIBILITIES:

Some of the responsibilities include,

  • As a member of the interdisciplinary team, the Peer support Care manager will mentor and provide recovery-based coaching to assist the consumer in learning self-management skills to pursue their individual health and wellness goals.
  • Provide individualized health coaching and teach skill building to independently manage in the community. This includes scheduling appointments, transporting consumers to scheduled appointments, assisting with linkage to specialists, assisting in obtaining medical equipment, supplies, and medications as needed.
  • Co-facilitate groups/classes to support improved health outcomes for consumers. Organize wellness activities.
  • Work closely with the interdisciplinary team to engage consumers in appropriate services and resources and assist with coordination of care.
  • Provide self-help recovery services and other peer wellness services.
