CPC's Adult Partial Care program (PC) is a community-based service that helps individuals living with serious mental illness or co-occurring mental health and substance use disorders to learn and enhance independent living skills. The Behavioral Health Clinician I provide individual, group, family, and/or substance abuse treatment for consumers who are diagnosed with co-occurring, mental health and substance abuse disorders. Works with a team to complete individual recovery planning and provides case management services. Completes Bio psychosocial Assessments and documents treatment progress in electronic clinical record.

Major Responsibilities/Performance Objectives:

Some of the responsibilities include,

• Complete an initial evaluation of the consumer’s needs based on the consumer’s expressed goals. This evaluation will include the following: presenting problem, medical, social, legal, educational/vocational, risk, drug/alcohol, nutrition, developmental and medication history, etc.
• Prepare an individual recovery plan with the consumer and interventions to meet those goals.
• Provide Treatment Modalities including skills training, relapse prevention, IMR, wellness, etc.
• Provide treatment addressing client’s cultural and religious issues as well as learning needs.
• Work with family to provide education and treatment when indicated.
• Assist with coordination of treatment services for assigned consumers.
• Utilize evidence-based treatment such as integrated treatment of co-occurring disorders, stages of change, psychiatric rehabilitation, etc. in direct practice.

Minimal Qualifications:

• Must possess a clinically oriented master’s degree.
• CADC, LSW, LAC and/or LCADC preferred.
• Training in Motivational Interviewing, integrated treatment of co-occurring disorders, and experience working with SPMI adults in outpatient as well as community settings.
