CPC Integrated Health's ICMS Program:

Integrated Case Management Services (ICMS) assists adults with serious and persistent mental illness and/or co-occurring substance use disorders increase independence and promote community integration. Case management services provide linkages to help secure necessary services and resources while maintaining recovery and a high quality of life.

About Our Role:

As part of our Integrated Case Management Services (ICMS) team, you'll play a pivotal role in linking individuals to services that promote stability, growth, and quality of life.

As a Behavioral Health Clinician, you'll:

• Collaborate with clients to develop and evaluate personalized goals.
• Conduct initial assessments and prepare comprehensive treatment plans.
• Provide case management services that foster independence and recovery.
• Document treatment progress, ensure continuity of care, and plan for discharge.

Annual Salary: $55,000 plus (based on years of experience and credentials.)

MINIMALQUALIFICATIONS:

• Must possess an active LSW or LAC.
• Must have prior experience providing treatment services to adults, children, or adolescents.
• A compassionate professional who thrives in a collaborative, dynamic environment.
• Experienced in mental health or co-occurring disorders, with a strong understanding of community-based support systems.
• COVID 19 vaccine, booster optional
