Care Manager - ICMS (Bilingual Spanish required)

CPC is hiring a case manager who will work as a member of a team responsible for coordinating services to and working with SMI consumers and their support network to promote wellness, recovery and productive lives

Location: Eatontown, NJ

About this Role

The Care Manager works in collaboration with the client in planning, implementing, and evaluating service provision to maximize the client's community adaptation, emotional well-being, medication response and general medical status to enhance stabilization and minimize clinical symptomatology. The Care Manager actively assists clients in navigating treatment needs and provides active care management including linkages to community resources.

Responsibilities

Some of the responsibilities of a Care Manager include,

  • Complete clinical documentation in a timely and effective manner. Comply with regulatory, Agency and funding guidelines regarding documentation.
  • Utilize evidenced based practices,
  • provide crisis intervention and link individuals to community support.
  • Assist other staff in completing assessments in a timely and comprehensive manner as required by CPC and regulatory agency policies.
  • Keep up to date on the use of the Agency's electronic health record.
  • Identify High Risk clients and work with team to provide safe and effective treatment outcomes.


REQUIRED QUALIFICATIONS:

Bilingual Spanish required

MINIMUL QUALIFICATIONS:

  • Bachelor's degree in related field and at least 2 years' experience.
  • A valid NJ driver's license with a satisfactory driving history is required.
