About the Program

The Adolescent Intensive Outpatient Program (IOP) serves youth ages 13-17 who are experiencing co-occurring mental health and substance use challenges. The program provides a structured, evidence-based therapeutic environment three afternoons per week. Services include individual, group, and family therapy, medication assessment and monitoring, case management, and crisis intervention. This IOP emphasizes a strengths-based, trauma-informed, and person-centered approach designed to help adolescents stabilize, build coping skills, and strengthen family and community supports.

Position Summary

The Care Manager provides comprehensive clinical services to adolescents and families enrolled in the IOP. Responsibilities include conducting individual and family sessions, facilitating therapeutic groups, coordinating care with prescribers and case managers, and participating in treatment team meetings. The Care Manager integrates evidence-based practices-including CBT, DBT-informed approaches, motivational interviewing, and relapse prevention-to address emotional regulation, substance use, and behavioral challenges.

Minimum Qualifications:

• Bachelor's degree in social work, Counseling, or related behavioral health field.
• CADC license.
• Minimum 2 years of experience working with children and adolescents with significant emotional, behavioral, or substance use challenges.
• Demonstrated expertise in group facilitation and crisis prevention/management.
• Strong clinical documentation and organizational skills.
• Flexible, compassionate, and able to "meet people where they are."

Preferred Qualifications:

• LCADC or CADC License.

• Master's degree in social work, Counseling, or related behavioral health field.
• 3+ years of experience in adolescent or co-occurring treatment settings.
• Experience in community mental health or intensive outpatient programming.
• Familiarity with electronic health records and outcome-based care.

Key Responsibilities:

• Provide individual, group, and family therapy for adolescents in the IOP.
• Conduct comprehensive assessments and develop individualized treatment plans.
• Facilitate dynamic group therapy sessions focusing on coping skills, emotional regulation, relapse prevention, and communication.
• Collaborate closely with psychiatrists, nurses, case managers, and crisis staff to ensure coordinated, integrated care.
• Maintain accurate and timely clinical documentation in accordance with agency and state standards.
• Utilize trauma-informed and culturally responsive approaches in all interactions.
• Participate in multidisciplinary case conferences, supervision, and staff meetings.
• Respond to and manage crises using de-escalation and crisis intervention techniques.
• Engage families in the treatment process and provide psychoeducation and support.
