CPC Integrated Health is seeking an organized and compassionate Wellness Care Manager to support the physical healthcare needs of individuals receiving behavioral health services. This position combines medical office responsibilities, wellness education, and care coordination to help clients access and follow through with recommended healthcare services.

Candidates with experience as a Medical Assistant, Patient Care Coordinator, or in a primary care or behavioral health setting are encouraged to apply.

Title: Wellness Care Manager

Location: Freehold, NJ

MINIMAL QUALIFICATIONS:

·Bachelor's degree in a health-related, human services, or related field.

·Minimum of one year of experience working with individuals, preferably in a healthcare or medical office setting.

·Experience as a Medical Assistant, Patient Care Coordinator, or in a similar healthcare role is highly desirable.

·Comfortable obtaining vital signs, gathering health information, and utilizing electronic health records, or willing to be trained.

·Strong interpersonal, organizational, and follow-through skills.

·Valid NJ driver's license and ability to travel between agency locations as needed.

PREFERRED QUALIFICATIONS:

·Bachelor’s degree in related field and two (2) years of experience working with people in a healthcare setting

MAJOR RESPONSIBILITIES/PERFORMANCE OBJECTIVES:

• Obtain and document medical histories, vital signs, height, weight, and BMI.
• Complete routine health, pain, nutrition, and wellness screenings.
• Review and update medications, allergies, and other health information in the Electronic Health Record (EHR).
• Assist with medication reconciliation in collaboration with prescribing providers.
• Coordinate medical referrals, appointments, and follow-up with primary care physicians and specialists.
• Communicate with outside medical providers to obtain records and coordinate client care.
• Assist clients in overcoming barriers to accessing healthcare, including transportation and scheduling.
• Provide basic wellness education and reinforce recommendations from healthcare providers.
• Collaborate with the treatment team to support clients' physical and behavioral healthcare needs.
• Maintain accurate and timely documentation.

• Good verbal and written communication skills.
• Ability to use technology, including electronic health record and a variety of software including Microsoft Office.
• Ability to travel to multiple locations.

The physical demands described here are representative of those that must be met by the employee to successfully perform the essential functions of this job.

The position requires prolonged sitting, some bending and stretching. Good eye-hand coordination and manual dexterity sufficient to operate standard office equipment is required. Employees must have normal range of hearing and eyesight to record, prepare, and communicate information pertaining to this position. The employee must occasionally lift and/or move objects up to 30 pounds.
