Position Summary:
The RN supports individuals receiving outpatient behavioral health services by coordinating care, providing health education, and assisting clients in reducing barriers to achieving their wellness goals. While this position requires RN licensure, it is primarily a case management and care coordination role rather than a traditional nursing position. The RN will collaborate with the prescriber, treatment team, and external providers to promote both physical and mental health recovery.

Minimum Requirement:

  • Current New Jersey RN license required.
  • Minimum one year of experience working as an RN.

Maximum Qualifications:

  • Experience working with individuals with mental health, co-occurring, or substance use disorders preferred.
  • Strong communication, organization, and documentation skills.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Commitment to promoting integrated and recovery-oriented care.

Responsibilities include:

  • Collaborate with clients, prescribers, and team members to coordinate care, provide education, and support individualized service plan goals.
  • Take vital signs and measure height, weight, and BMI; record results in the electronic health record (EHR).
  • Complete or assist clients in completing health, pain, and nutritional screenings; review results and make recommendations in collaboration with the supervisor and prescriber.
  • Provide medical case management, including scheduling and follow-up for medical appointments, and assist with referrals and care coordination.
  • Administer and document long-acting injectable medications as prescribed; maintain medication logs.
  • Assist with medication prior authorizations and other related administrative tasks.
  • Support the medication line ("Med Line") process and ensure accurate documentation.
  • Respond to emergencies within the program and provide support within scope of practice.
  • Promote wellness, healthy lifestyle choices, and self-management of medical and behavioral health conditions.

The physical demands described here are representative of those that must be met by the employee to successfully perform the essential functions of this job.

The position requires prolonged sitting, some bending and stretching. Good eye-hand coordination and manual dexterity sufficient to operate standard office equipment is required. Employees must have normal range of hearing and eyesight to record, prepare, and communicate information pertaining to this position. The employee must occasionally lift and/or move objects up to 30 pounds.
