Job Opportunity: Care Manager - BRIDGE Program

Location: Monmouth County, NJ

We are seeking a compassionate and dedicated care manager to join a new program called BRIDGE: Building Resources and Integration for Dedicated Growth and Empowerment. BRIDGE is aimed at improving the quality of care for individuals with Serious Mental Illness (SMI) in Monmouth County. This innovative initiative focuses on enhancing care coordination, facilitating smooth transitions of care and connecting individuals with necessary services to prevent gaps in care.

Daytime and Evening Hours

Minimal Qualifications:

  • BA Degree in Mental health or related field.
  • Ability to communicate effectively and clearly in written and spoken English.
Preferred Qualifications:
  • BA Degree in Mental Health or related field with mental health experience
  • 3 years of experience or Master's Degree in related field.

Key Responsibilities:

  • Visit Monmouth County hospitals, acute care programs and primary care practices to provide individuals with real-time education and service linkage.
  • Ensure individuals with SMI receive comprehensive, integrated and equitable services from a whole health perspective.
  • Initiate service linkages based on the individual's expressed preferences and the needs identified by referring healthcare providers.
  • Schedule primary and specialty care appointments for clients, ensuring they have access to the services they need.
  • Provide transportation to and from medical and other necessary appointments.
  • Serve as an advocate for clients during medical visits and appointments.
  • Proactively connect clients with vital community resources, ensuring their needs are met in a timely and coordinated manner.
