CPC Integrated Health's ICMS Program:

Integrated Case Management Services (ICMS) assists adults with serious and persistent mental illness and/or co-occurring substance use disorders increase independence and promote community integration. Case management services provide linkages to help secure necessary services and resources while maintaining recovery and a high quality of life.

About Our Role:

The Behavioral Health Clinician works in collaboration with the consumer in planning, implementing, and evaluating service provisions to maximize the client's independence and success in the community.  The Behavioral Health Clinician completes initial assessments, prepares a treatment plan; provides case management services; documents ongoing treatment and discharge planning.

This is a community-based position. 

Annual Salary: $55,000 plus (based on years of experience and credentials.)

MINIMALQUALIFICATIONS:

• Must possess terminal NJ license and Master's Degree in clinically related field such as social work or counseling. 
• Must minimally possess an active LSW or LAC.
• Must have prior experience providing treatment services to adults, children, or adolescents.
• COVID 19 vaccine, booster optional

PREFERRED QUALIFICATIONS: 

• Experience with evidence-based practices.
